Office Visit Fee Refund Policy
A $100 Office Visit Fee is charged for patients who schedule and attend a single treatment appointment. This fee is applied to the cost of the appointment.
This fee covers the provider's time, consultation, treatment planning, administrative costs, and appointment reservation.
The $100 Office Visit Fee is non-refundable under all circumstances, including but not limited to:
- Appointment cancellations or rescheduling requests.
- Failure to attend a scheduled appointment (no-show).
- Early termination of treatment.
- Dissatisfaction with treatment outcomes.
- Changes to a patient's treatment plan.
By scheduling and attending a qualifying appointment, patients acknowledge and agree to this non-refundable fee policy.
